John, Any form mailing cgi program that I have ever worked with will check for the domain the form is submitted from. This is a security feature that prevents spammers from mailing millions of pieces of mail using a mail script - the request to mail it must come only from a particular Web space. I don't know of any script that would honor a form processing request that was submitted by email from a users computer.
You might test using a "mailto" method in the form tag instead of a script method. And you can set up the options on a form to mail the entire PDF document as form results rather than only the form data.
