I haven't done this directly; however, Blue Sky Software (RoboHelp) has two products that may help: Help Decompiler (turns your HLP file into a Word document; however, all the links are removed - this is for people who don't have the source files.
The other product is Help2HTML, the new version of which reported supports conversion of SHED graphics into image maps. This might be a way, if either 1) the client would be happy with an HTML solution or 2) there is some tool that will take HTML and turn it into PDF.
