The XTension for 4.0.4 is called PrintRGB XT and is available at the Quark download section on their web site <www.quark.com>. However, I think you would be better off downloading the free update to QuarkXPress 4.1, which has the feature built-in and does not require the XT.
The Print RGB XT ONLY allows XPress to print RGB (rather than CMYK), the other options are part of the PDF Filter XT and the PS print driver. I haven't had any trouble exporting to PDF with the 4.0.4 version or the newest version that comes with the 4.1 update.

You don't need to keep transfer functions unless you actually set them in Photoshop. Making a clipping path does not necessarily require transfer functions. However, having the setting on should not affect anything since you are (most likely) not using any transfer functions, so I would not change the setting.
