There is also a product that Microsoft has for win95 machines....Its called the HTML printer driver....It is inside the powertoy.exe file that was built by the ms development team......Not officially supported though.
Just start reader, pick the printer and print.....

There is a program called Net-It which 'prints' to an HTML output (no plug-ins, etc. needed in the browser) -- but the output is only readable with browsers using Java.
One could open the PDF in Reader, then print to the Net-It and the output would be html for a web site.
